Care That Brings
Independence Home
Prestige Home Health delivers person-centered, community-based support for individuals with disabilities and older adults across Minnesota — in the comfort of their own homes.
"We tirelessly work to support people with disabilities to live and work in communities of their choice."
Empowering Lives Through Community-Based Care
Founded in Minnesota, Prestige Home Health exists to give individuals with disabilities and older adults the support they need to live full, self-directed lives — not in institutions, but in their own homes and communities.
As a licensed 245D provider, we work closely with county case managers, care coordinators, and families to build personalized plans that reflect each person's goals, strengths, and preferences.
