Our Purpose

Founded in Minnesota, Prestige Home Health exists to give individuals with disabilities and older adults the support they need to live full, self-directed lives — not in institutions, but in their own homes and communities.

As a licensed 245D provider, we work closely with county case managers, care coordinators, and families to build personalized plans that reflect each person's goals, strengths, and preferences.
